Please Note: This form should only be used when registering for self-paced computer based training.

The Department listed below requests the purchase of credits for self-paced computer based training. We understand and acknowledge our department will be charged for this purchase and below we are issuing the Blanket Release Number (formally known as a PG number) in order to obtain the necessary purchase funds.

The table below shows the price structure for credit purchases.

Please Note: Our self-paced computer based training is sold in bundles of 3, giving the user access to all 3 levels (Basic, Intermediate and Advanced) for a total of $25.50 for a 1 year subscription.
